Initiation of Injectable Progesterone Therapy in Woman with Previous Pre-term Births - The percentage of women 15-45 years of age with evidence of a previous pre-term singleton birth event (140/90) during the measurement year.Denominator: Initiation Phase – Members 6-12 who were newly prescribed a ADHD medication between March 1st of the Prior Year through February 28th of the Measurement Year.Numerator: Continuation and Maintenance Phase – Member should receive TWO outpatient or follow up visits with a practitioner with prescribing authority within 31-300 days (9 months) after the Initiation Phase.Follow-up Care for Children Prescribed ADHD Medication Continuation & Maintenance Phase - The percentage of children 6-12 who were newly prescribed ADHD medication who had TWO follow-up care visits within 300 days after the date the medication was dispensed.Denominator: Initiation Phase – Members 6-12 who were newly prescribed a ADHD medication between March 1st through February 28th of the Measurement Year.Numerator: Initiation Phase – Member should receive an outpatient or follow up visit with a practitioner with prescribing authority, within 30 days after the date the medication was prescribed.Follow-up Care for Children Prescribed ADHD Medication Initiation Phase - The percentage of children 6-12 who were newly prescribed ADHD medication who had a follow-up care visit within 30 days of when the medicine was dispensed.Denominator: Members 3– 21 years of age as of December 31, the measurement year.Numerator: At least one comprehensive well-care visit with a PCP or an OB/GYN practitioner during the measurement year.This measure is a combination measure that replaces the former “Well-Child Visits in the Third, Fourth, Fifth and Sixth Years of Life” and “Adolescent Well-Care Visits” HEDIS measures.Child and Adolescent Well-Care Visits (WCV)- The percentage of enrolled members 3-21 years of age who had at least one comprehensive well-care visit with a PCP or OB/GYN practitioner during the measurement year.To avoid double counting events when only assessing administrative data or when combining administrative and medical record data, all events must be at least 14 days apart Denominator: Rate 2: Date of second birthday plus 180 days.Denominator: Rate 1: Calculate the 15-month birthday as the first birthday plus 91 days.Numerator: Rate 2: Two or more well-child visits between the child’s 15-month birthday and 30-month birthday.Numerator: Rate 1: six or more well-child visits on or before the 15 month birthday.Children who turned 30 months of age during the measurement year.
